You have to consider that they were on the island actually just for a couple of months, afterwards he was in "the real world" for 3 years where he probably put all the weight back on, that he lost during that time. which was probably not that much since as someone earlier pointed out he was in charge of the food. And after they came back to the island there wasnt much time that went by yet. you have to consider that the narrativ time is way shorter than the time the show has been on tv
